This weekend I am in Denver, Colorado, as one of the first of four physicians in the world to be trained in SmartLipo® HighDef®. I am attending the first course taught by Dr. John Millard, who introduced the technique of HighDef® liposculpture to the United States. The technique was originally developed by Dr. Alfredo Hoyos of Colombia. With the introduction of the latest generation SmartLipo® Triplex machine, laser liposculpture technology can now be merged with the the truest technique of body sculpturing yet developed, HighDef® liposculpture. HighDef® liposculture is a meticulous technique of marking the anatomical structure of your muscles, followed by true contouring of the overlying fat to reveal the underlying muscle anatomy. In this technique fat is not only removed generally as in other forms of liposuction, but the fat is selectively contoured following the contour of the underlying muscles, removing more in some areas than in others, to create a sculpted form. In some situations some areas may have no fat removed, or one may even add fat to an area to create the desired contour and ideal anatomic shape.
The technique of SmartLipo® HighDef® can be used in conjunction with Vaser® HighDef® to enhance results. Though I have always made the effort to enhance the appearance of the underlying musculture for my patients, the combination of SmartLipo® technology with the HighDef® technique, takes the art of liposculpture to a whole new level.
HighDef® liposculpture is most commonly known as a way of creating a six-pack appearance of the abdomen, but in fat can be used anywhere in the body to sculpt and reveal the definition of the underlying muscles. So HighDef® liposculpture is also popular to enhance the appearance of the arms, back, legs, buttocks, and even the breasts.

Questions regarding breast augmentation with fat - is it safe?

Obviously the most important question to ask for any plastic surgery procedure is whether or not the procedure is safe. The main concern of plastic surgeons with regard to fat placed in the breast is whether this has any effect on the detection of breast cancer with mammograms, and whether fat grafting can cause breast cancer. After more than 10 years of studying this question, the answer to both questions appears to be favorable, particularly if meticulous micro-fat grafting procedures are used. In a study of 50 breast augmentation cases with fat Khouri et al revealed no difficulties with mammographic interpretation and no evidence of increased incidence of breast cancer. Particularly when compared to some of the problems that can occur with breast implants such as capsular contracture, fat grafting for breast augmentation appears to be a safe procedure.

Stem Cell Enhanced Fat for Facial Rejuvenation and Breast Augmentation

The Bougainvillea Clinique is now the first plastic and reconstructive surgery center in Central Florida to offer stem cell enhanced fat grafting as well as fat derived stem cell treatments for cosmetic and reconstructive surgery.

This cutting edge technique utilizes stem cells and other regenerative cells, collectively known as Adipose Derived Regenerative Cells (ARDC’s) separated from your own fat (removed by liposuction) and then used to enhance traditional fat grafting techniques.

Fat has been shown to be the richest source of adult stem cells, however the cells must be separated from the fat in order to fully realize their potential.

To isolate the ARDC cells, first liposuction is performed, and then some of the fat is uniquely treated using patented technology to produce a purified mixture of these ARDC’s.

This mixture can then be used in a number of ways, either added back to fat for grafting to create a ‘supercharged’ fat graft, or by itself to be injected under the skin to improve skin quality and wrinkles such as crow’s feet. Other cosmetic uses are breast augmentation, treatment of acne scars, and improvement of the appearance of the aging hand.

This stem cell or ARDC enriched technique can also be used for breast reconstruction after breast cancer, and for treatment of problem wounds such as after radiation treatment for cancer.

It is also now possible to freeze your own fat and store it to be used in the future for cosmetic, reconstructive and other potential medical uses. ARDC treatments are now being studied for many other medical conditions that require restoration of damaged tissue, such as after heart attacks, kidney injury, bone and nerve injuries.

Although traditional fat grafting obviously includes stem cells naturally found in the grafted fat, creating a supercharged fat graft by adding more of the stem cells or ARDC’s to the fat has been shown to improve the quality and survival of the fat graft by up to 50%. Another benefit of separating out the stem cells is that these cells can now be used by themselves to improve the quality of issues, particularly skin, without adding any volume of fat to the area if this is not desired. Body bronzing (air brush tanning) is a safe way to get a deep, bronze tan that is applied by our tanning expert. Restoring the Volume of Youth - The Liquid Facelift

The last 10-15 years have seen a dramatic change in how plastic surgeons approach the rejuvenation of the aging face. In addition to the traditional facelift procedures, surgeons are now paying attention to restoring a more natural youthful appearance, which frequently requires restoring volume, to areas of the face that have lost volume. These areas include the temples and jawline, the cheeks and around the eyes.

In my Orlando area plastic surgery practice, I have been utilizing volume enhancement techniques for facial rejuvenation patients for over 15 years. These techniques include facial implants, fat grafting, and more recently injectable materials such as Sculptratm and Radiessetm. Particularly in younger patients, volume restoration is all that is needed to maintain a more youthful appearance, delaying the need for a facelift procedure. In older patients with more volume loss as well as sagging of the tissues such as the cheek tissues, procedures such as a facelift and midface lift, may be combined with volume enhancement techniques.

In the first example shown a Quicklifttm short scar facelift technique was combined with lipostructure fat injection techniques to the cheeks and nasolabial folds for a more balanced and youthful appearance.

In the second case, only Lipostructuretm fat injection in combination with the Obagitm Skin Program and Blue Peel, were used to restore a more youthful and radiant appearance.

Safety of Saline and Gel Breast Implants - get correct information

Occasionally we call other plastic surgeons' offices to find out what kind of information they provide to their patients. Unfortunately, we have found that the receptionists at many offices often provide very poor, or even false information with regard to the safety of breast implants. We are not sure whether the information they provide reflects the opinion of their surgeon, or whether they are just poorly informed. We suspect that they are at least reflecting the bias of their surgeon. This is particularly the case with regard to saline and gel breast implants. One office we called indicated that they never do gel implants because they are bad, and another office told us they never do saline implants because they are bad. The reasons they provided where even more interesting. The office where the surgeon did not perform gel implants, informed us that gel implants were bad because the gel goes all over the body and makes people sick. Really now, do you honestly believe the FDA, after all the publicity, would have released gel breast implants back on the market (after 14 years of intensive study) if this were true? Clearly not. The office whose surgeon does not do saline implants, said saline implants were bad because when they leak, they just go flat. Pretty obvious, but why is this bad, when it is a simple matter to replace a leaking implant.

The reality is that gel and saline implants are both artificial devices, both have advantages and disadvantages depending on the individual patient, and neither implant is 'bad' or unsafe. Yes, there are potential complications with both gel and saline implants such as capsular contracture (hardening), but neither implant poses general health risks of any kind to the patient.

At the Bougainvillea Clinique we offer saline and gel breast implants, and we make recommendations based on your body type and the size, shape and feel that you want from your breast augmentation procedure. We explain the advantages and disadvantages of saline versus gel implants, and let you make the final decision. So be cautious regarding some of the misleading information regarding breast implants out there on the internet, and unfortunately from some surgeons' offices. Feel free to schedule a complimentary consultation with us, where we promise to provide honest, straight forward information based on my experience with over 4000 breast implant procedures, both saline and gel. Remember we are also the first, and presently the only practice in Central Florida to offer the Vectra 3-D Breast Imaging service, to give you a state-of-the-art look at how your breasts will look after the procedure, and allow you to make the best informed decision possible with regard to the size implant you should choose.

Breast Augmentation anesthesia options

At the Bougainvillea Clinique we offer our breast implant patients many options and choices. We provide saline and gel implants, under the muscle and over the muscle placement, and all the various incisions, including under the breast (inframammary), under the nipple (areolar), through the belly button (TUBA) and through the armpit (transaxillary). One of the questions we are sometimes asked, pertains to the options available for anaesthesia. Like any other cosmetic surgery procedure, breast augmentation can be performed under local anaesthesia, local with sedation (twilight) or general anaesthetic. Some surgeons place a lot of emphasis on the fact that they do breast augmentation under pure local anaesthetic, and although we will provide this if our patients specifically request it, it is not our recommendation for a number of reasons. Before I explain these reasons, I would like to emphasize that all the surgeons in the Orlando area who strongly promote local anaesthetic, do not have fully accredited and licensed surgical facilities, and as such, it is illegal for them to provide any services under intravenous sedation or general anaesthetic in the State of Florida. At the Bougainvillea Clinique, we have the fully state and federally certified Aesthetic Surgery Center of Winter Park, so we are able to make recommendations based on what we think is best for you, and not based on the limited options that some other surgeons are able to offer.

Breast augmentation is a relatively short and quick procedure, so our objective is to make sure that your procedure is done safely, with minimal discomfort, and a rapid recovery. After performing more than 4000 breast augmentation procedures without a single anaesthesia complication, I have found that the deep sedation and general anaesthesia techniques that we use, combined with local anaesthetic works best for our patients. The technique is similar to the technique used thousands of times everyday in the US for short surgical and diagnostic procedures such as colonoscopy.

The combination of short acting sedation and anaesthetic medications, combined with local anaesthetic in the area around the breast, allows the procedure to be performed with zero discomfort during the procedure, quick recovery and rapid return to normal activities.

Another reason that some other practitioners promote as a reason to use local anaesthetic only, is that this allows you to see the size of the implants during the procedure. I believe this is very misleading, as it requires large amounts of local anaesthetic fluid in the breast tissues to perform the procedure purely under local anaesthetic. This fluid distorts the breast shape and size, so you will not be able to accurately decide on the size of the implant with this procedure.

Finally, the use of local anaesthetic only for breast augmentation may require high doses of local anaesthetic to make the patient comfortable, and may need to be placed in an area of rapid absorption, particularly for under the muscle placement of the implant. There have been reports of overdosage of local anaesthetic in situations like this, which can result in complications such as seizures. Yes, breast augmentation can be performed under local anaesthetic only, but it may not be the simple, comfortable and safe procedure that some surgeons would have you believe. If a surgeon recommends breast augmentation under local anaesthetic alone, be sure that his facility is licensed to provide all types of anaesthesia, including general anaesthesia. Be certain that he is recommending local anaesthetic as an option, and not the only option he can provide.

Mammograms are a critically important means of screening for breast cancer - both prior to your breast augmentation surgery and afterwards. A screening mammogram is recommended every one to two years after age 40, dependent upon on your risk factors.

Get Routine Mammograms
Breast implants can inhibit the degree to which mammograms identify breast cancer, more or less depending upon how your implants are placed. Even so, research does show that mammograms are an effective way to screen for breast cancer - even in women who have had their breasts augmented.

Locate an Experienced Facility
Once your implants are in place, locate a breast screening facility that performs a large number of mammograms annually - and includes a significant number of women with breast implants. For these women, the mammogram procedure is a little different, and the results are trickier to interpret. You need an experienced radiologist that knows what to look for. With silicone implants, you should have an additional imaging exam (MRI) every three years after implant placement and every two years thereafter to exam the implants for rupture sites.

Breast Augmentation and Pregnancy

A pregnant woman goes through numerous changes during nine months of pregnancy. Even after the baby is born, her body continues to change. For most new mothers, weight loss is a prime concern, prompting new mothers to seek breast augmentation as a solution to regaining their pre-pregnancy attractiveness.

Breast cancer is the second most common type of cancer after lung cancer, and the most common female cancer. Screening, along with early detection and treatment, is crucial to defending yourself against this insidious killer. Physical examination, mammography and at-home breast self examination are the keys to early detection.

How might breast implants interfere with cancer screening? In 2007, 399,440 breast implants surgeries were performed, according to The American Society of Plastic Surgeons. Studies show that breast implants do not interfere with treatment for breast cancer.

Today's advanced cosmetic surgeons use the latest saline and silicone gel breast implants, prompting the creation of customized X-ray techniques like the Eklund view to ensure breast implants do not interfere with mammography readings. New 'Gummy Bear' Breast Implants on the horizon

Having completed fairly extensive studies in Europe and the US, both Mentor and Allergan are hoping for FDA approval of their more cohesive 'gummy-bear' implants. These implants appear to have significant advantages particularly in terms of wrinkling, over saline implants and presently available round gel implants. For very slim individuals, with very little soft tissue in the breast area, particularly on the side, this will be a significant improvement. These new implants are also textured and shaped to provide a natural anatomic shape to the breast. One slight disadvantage is that they may not move quite as naturally as the softer smooth round gel implants that are presently the most popular gel implants in the United States. Because these implants wrinkle less, it is also likely that they last longer than implants that wrinkle more.

Though ideal for some patients, these implants are not ideal for everyone. They will be most suitable for those who are looking for the most natural, and a modest increase in breast size. Those who are seeking larger augmentations will not be suitable candidates for these implants. Presently about 30% of implants used in Europe are the shaped 'gummy-bear' gels. TUBA

TUBA stands for Transumbilical Breast Augmentation, a name that refers to where the incision is located. Instead of placing incisions in the armpits, around the areola, or underneath the breasts, a TUBA procedure uses a small incision inside the navel.

Dr. Hartog uses an endoscope -- a thin hollow tube with a camera and light at the front end. Being connected to a computer, it sends images to the monitor from wherever it is positioned. This gives large and clear images to Dr. Hartog of the work he is doing.

He inserts the endoscope through the incision and beneath the skin up to where the implant will be placed. A 'sizer' (a temporary implant) is passed through this tunnel, put into place and expanded to create the space for the implant. The sizer is removed and the empty implant is then passed through the tunnel and put into place. It is filled and the incision is closed with dissolvable stitches.

Which Breast Augmentation Incision Is Right for You?

Now that you've decided to go ahead with your breast augmentation, there are a number of decisions you have to make about how you want the procedure done. Once you have a rough idea of the size of the implant you want and whether you want to have silicone gel implants or saline implants, your next important decision is which incision is the best one for your breast augmentation surgery. There are four common incision sites for breast augmentation surgery:

Inframammary--in the fold under the breast
Periareolar--just below the nipple/areola
Transaxillary--in the armpit
Transumbilical (TUBA)--in the belly button

If you have decided to get a silicone gel implant, you cannot have breast augmentation via the transaxillary or transumbilical incisions. Otherwise, the main factor to consider in making this decision is about where you would prefer to have your scars from the operation.

For most women, the inframammary incision is the best option. It allows for maximum flexibility in terms of implant insertion, is least likely to result in complications like interference with breastfeeding or loss of nipple sensitivity, and is generally well concealed.

Women with a history raised or highly visible scars should consider the periareolar incision. The incision is well concealed in the natural transition between the breast and the areola, and the scars tend to heal slightly better.

Women who want to make sure they have no scars on their breasts can opt for either the transaxillary or transumbilical incision. These procedures are performed with an endoscope, a long narrow tube that allows Dr. Hartog to perform the surgery using a distant, small incision. Since these incisions are smaller, so are the scars. On the other hand, the transaxillary incision site in particular is in a prominent location, so if you tend to have highly visible scars, it might not be a good option. The main drawback to these endoscopic procedures is that there is a higher incidence of asymmetric results.

Making Decisions about Breast Enlargement: Size

Since women have been receiving breast enlargements, they have been coming up with different bits of folk wisdom for getting an idea about the size of implant they wanted. If you look at discussion forums for women who are considering or have gotten breast enlargement surgery, you can find them by the dozen.

The most common folk method is getting a bra in the size you hope to get as a result of your breast enlargement surgery, then seeing how much rice, birdseed, or couscous it takes to fill out the bra around your breast. Once you have a basic idea of the size, you can try to experiment with results by filling a baggie with that amount of material and placing it under your breast and wearing it around all day. This is roughly the same idea as that behind breast implant sizers that doctors have used in the past during consultations. It has the disadvantage of being less calibrated and uniform, but the advantage that you can wear it around all day to learn what life might be like after implants. You can try on every outfit in your wardrobe or even at the store to decide how you feel about the results.

However, the results of this test--and even of the implant sizers--are not directly comparable to the results you will achieve from your breast augmentation. Why? The breast implant is not placed under your breast in the way the rice or other material is. It is placed behind the breast and pushes up and outward on the breast at the same time. And, over time, your breast will conform around the implant, incorporating it completely into the shape. Once you understand what your actual results will be like, you can continue to use the rice test to consider what outfits you might light to wear after your operation.

One important test to do on your own: if you are considering saline implants, try wearing a bra with baggies containing the approximate amount of water that will be in your implants to determine whether they feel comfortable or not. Saline is dense, and some women with large implants experience lower back pain as a result. If the water seems too heavy, consider silicone gel implants, which are less dense.

The Bougainvillea Clinique is pleased to announce that we are the first in Central Florida to introduce the latest in laser body contouring, the new SmartLipo MPX technology. SmartLipo MPX is the second generation laser from Cynosure that is able to melt fat and help enhance skin contraction. A significant improvement in power and versatility over the first generation SmartLipo laser, the MPX uses two lasers in one to melt fat more efficiently, and get even more skin retraction than was apparently possible previously. Using a small laser fibre the area is treated twice, first in the deeper layer of fat, and then again directly under the skin.
